Investigate and identify areas of improvement and support to the clients
Organize and conduct social activities to support clients.
Assessing clients and gathering relevant information.
Contacting and making referrals to other agencies and services.
Organize support groups or counsel family members to assist them in understanding, dealing with, and supporting the client or patient.
